We are a 200-bed long term care/short term care rehab facility serving all the needs of the elderly: physical, spiritual, psychological and social.

rovidence Rest Adult Day Health Care is a medical program dedicated to providing quality care for the chronically ill and frail aging population. Our mission is to provide our registrants with a warm and caring environment where they can enjoy the companionship of others while participating in fun activities that promote wellness and independence.

Through the implementation of individualized care plans our nurses, dietitians, social workers and rehabilitation therapists work closely with registrants, families and health care providers to provide services that can help to promote optimum health status and prevent deterioration.

Our program provides the support necessary to assist your family member to continue living in your community. Providence Rest Adult Day Health offers a safe alternative for the older adults who chose to live at home while giving peace of mind to caring families as well as the registrants themselves.

Under the care of your community physician, the following services are available:

	CASE MANAGEMENT
	REHABILITATION THERAPIES (PHYSICAL, OCCUPATIONAL, SPEECH)

	PODIATRY CARE
	BEAUTY PARLOR SERVICES
	RELIGIOUS SERVICES AND COUNSELING
	PERSONAL CARE

Admission Criteria		Payment Option		Program Hours
Older adults who are physically disabled, chronically ill, frail and in need of assistance with activities of daily living.		Private pay is welcomed and Medicaid is accepted for those who are eligible.		Our program is opened seven days a week, between the hours of 8 am and 4 pm.
