  Application form  

   rovidence Rest welcomes those who will benefit from the kind of skilled nursing we can provide.  Our focus is on the geriatric population and older adults who are in need of continuous skilled services.  We consider and review all the applications we receive for placement.  To start the process, you can forward a PRI and Screen directly to our admissions office.  We will provide an application.  We will also arrange for pre-admission tours of the facility and interviews upon request.

    Our Social Service Department will contact you when you apply to the Home for admission.  They will request insurance information and other documentation.  We admit most of our residents from acute care (hospital) settings, and at times, from other long term care facilities or private homes.  Residents admitted to our Short Term Rehab Program usually stay from three weeks to three months based on their individual needs. 

    Respite stays requires a reservation, preliminary application and medical form.  Private pay stays require a deposit.  Our respite space is limited, and rooms are booked quickly.  There is a two-day stay requirement for the respite program.  Please contact our admissions office to obtain an application and brochure.

    Providence Rest, in affiliation with Calvary Hospital, offers in-patient Hospice Care.

    Our Social Service staff helps inform, guide, and reassure our residents from the moment they consider applying for admission to Providence Rest.  Resident and family education is an important component of our social service program.  The staff provides support and coping techniques to new residents and their families.  A support group for family members encourages networking as well as providing a shoulder to lean on. 

    For short-term residents who will be returning home, we provide appropriate discharge planning using community agencies, to ensure access to needed services. 

    Providence Rest will ensure that all residents are afforded their rights to a dignified existence, self-determination, respect, full recognition of their individuality, consideration and privacy in treatment and care for personal needs.  The Home shall protect and promote the rights of each resident, and shall encourage and assist each resident in the fullest possible exercise of these rights. 

    New York State and Federal Law prohibit discrimination in any form on the basis of gender, race, creed, color, national origin, handicap or source of payment in any program or activity receiving any assistance or support from public money.
