e offer a comprehensive team approach to meet the specific needs of individuals.  Through physical, occupational, and speech therapy, residents attain their maximum level of functioning skills that they need to return to home, and/or to their previous function in the community.  Our programs are specially designed to assist older adults recovering from compromising medical conditions, most of who were recently hospitalized and has become unable to safely return to their home environment.  Arrangements are made prior to discharge for necessary equipment and devices that would assist the resident at home.  Family/caregiver training is given and the therapists coordinate with the home-care services for a smooth transition to home.
